A Care Coordinator organises and manages care services for individuals, ensuring they receive the right level of support at the right time. This is a more administrative and managerial role that involves assessing needs, creating and updating care plans, scheduling staff, and maintaining excellent communication between clients, families, and care teams. Strong organisational and people skills are essential.
